I, the "Guardian", for and in consideration of the "Minor", under the terms of this Agreement and sufficiency of which is hereby acknowledged, do hereby release and forever discharge Athlete's True Performance Institute, LLC of Evansville, Indiana (Hereinafter the "Releasee") from any and all claims, demands, damages, actions, causes of action or suits which I now have or may hereafter have, arising out of or in any way relating to any and all injuries and damages of any kind.*

I also hereby grant Athlete's True Performance Institute, LLC and their agents the absolute right and permission to use photographic portraits, pictures, digital images of the Minor, or in which the Minor may be included in whole or part, or reproductions thereof in color or otherwise for any lawful purpose whatsoever, including but not limited to use in any social media publication or on the company websites, without payment or any other consideration. *
